The Giant Cave Maggot is found firstly in the caves South of Tulimshar, where an encounter is required to complete the Newbie Quests. It is slow-moving, but has a powerful bite, and has a lot of HP, meaning it will take some time for the newer players to kill it.
The drop of this Giant Cave Maggot is needed at least once in game, which is in the aforementioned Quests. It is a very heavy item, and will prevent you from being able to get healed naturally. Keep this in mind when you do this quest, as you may need a friend to help you. Sema is the NPC who needs it, so unless for some reason you are collecting them for fun, be sure to give it to Sema as soon as you get it.
